1. Having read the
Official FBFP 2012 Electric Elite Invitational Rules and completed the below
entry form and paid the per event and a one time $80 team fee, I/we agree to abide by all conditions stated therein.
2. Liability Release: By signing this agreement, I/we hereby release Akron City Watershed, State of Ohio, FBFP 2012, Rory Franks and any of it's officer’s, agents, employees and sponsors from any and all damages, claims, costs or expenses relating to injury or even death, of any persons, property, or character which we may sustain or cause by reason of our participation in or in connection with this tournament. I/we further authorize the use of any images or likeness of our person and names.
3. I/ We have
